Functional Capacity Assessment

​What to expect:

Request

Occupational Therapists complete functional capacity assessment when requested to describe a person's level of function. These reports may include request for NDIS SIL/SDA, Department of Housing, Sensory Profile Assessments and plan review assessments/ change of circumstances. 

Initial Assessment

 Initial appointment can be up to 2 hours. They will assess your function in various domains, complete clinical observations and standardised assessments to obtain information on your level of function. 

Follow Up Assessments

Follow up appointments can be booked in if required to obtain further information on your functional capacity e.g. task analysis in different environments. 

Liaisons

 Liaison with your specialist team/support workers/relevant care team to obtain further relevant information if required. 

Comprehensive Report

A comprehensive and detailed report is completed, outlining your functional capacity with recommendations. This is then provided to you or your coordinator/case worker for approval.
